Central Park West Primary Care and Cardiology Physicians is a primary care and internal medicine practice serving adults on the Upper West Side of Manhattan, New York. Under the leadership of Haleh Milani, MD, FACC, the multi-specialty practice strives to deliver attentive, personalized care to improve patients’ cardiovascular health and quality of life. 

Dr. Milani and her dedicated team of physicians are committed to your health journey and focus on helping you achieve excellent long-term health. Through cardiac imaging, stress tests, and preventive cardiology, the providers can detect, monitor, and treat conditions that affect your heart so you can avoid future complications. 

The practice also uses the concierge model of care, which allows the team to better serve you and address your health needs. They make themselves available to answer your questions, spend time talking with you about your condition, and offer exceptional attention-to-detail when it comes to your treatment.  

As the medical director, Dr. Milani ensures patients receive the highest quality of care. She is  a board-certified internal medicine physician who has fellowship training in cardiovascular medicine from SUNY Downstate Health Sciences University. She also holds sub-specialty certification in cardiovascular diseases.

Honored by the medical community, Dr. Milani has received several awards in the past, including “Top Doctors” recognition by NY Top Docs and Castle Connolly. She was also awarded Vitals’ “Compassionate Doctor Award” and “Patients’ Choice Award.” In addition to helping patients at her practice, she is also affiliated with Cornell University, Columbia University, and Mount Sinai in New York.

Dr. Milani is active in charitable societies and organizations, including Doctors Without Borders, St. Jude Children’s Hospital and the American Red Cross. She is also an ambassador for the global fight against hunger and improved health around the world.

Dr. Milani volunteers her time to educating the doctors of tomorrow by teaching medical students/residents/cardiologists-in-training at the teaching hospital of Columbia University Medical Center. Outside of her clinical practice, Dr. Milani is a research investigator on numerous clinical trials. Dr. Milani lives on the upper West Side with her husband and two young children having moved from England years ago to pursue her medical career in midtown Manhattan, NYC.

Dr. Elaine Kang is board certified in Family Medicine. She graduated from New York Medical College in 2006 and she completed her residency at Beth Israel Residency in Urban Family Practice, New York, NY. Dr. Kang is a leader in the field of women's health. She completed the Physicians for Reproductive Health and Choice Leadership Training Academy to provide advocacy and leadership in the reproductive health field. In addition to her work at Early Options, she currently works in urgent care with CItyMD, in the Emergency Department at North Central Bronx and Jacobi, and provides coverage with the Institute for Family Health, precepting residents and providing healthcare for the homeless.
